Guest Liability Waiver


In consideration for providing haircuts, color and any salon services, by completing this form and submitting below you agree to accept the terms to ensure safety compliance in helping the staff and company known as the Cellar mitigate as many risks as it relates to COVID-19 and the contraction of the virus. While we are taking your safety and that of our staff very seriously by employing new safety and sanitation initiatives, enhanced use of PPE, extra cleaning along with customer covid checks, we cannot guarantee that any of these measures will completely protect you from contracting COVID-19.

 

Knowing the potential risks involved given this uncertain time and in consideration of the extra efforts that are undertaken by the Cellar staff, I agree to waive claims against the salon as it relates to usual business activity and possible and or potential transmission of Covid 19 as stated in Toronto Public Health communications. Should you have any concerns, please contact the salon to find out the extra steps we are taking to help keep you safe.

Do you currently have one or more of the COVID-19 symptoms below that are new or worsening? fever and/or chills, cough or barking cough (croup), shortness of breath, decrease or loss of smell or taste, fatigue and/or malaise, nausea/vomiting, and/or diarrhea
Do you live with someone who has been told by a doctor, health care provider, or public health unit that they should currently be isolating? If you are fully immunized* or have tested positive for COVID-19 in the last 90 days and since been cleared* , select “No.”
Have you returned from travel (including travel within Canada) in the last 14 days OR had *exposure to a confirmed positive case of COVID-19?
Has a doctor, health care provider, or public health unit told you that you should currently be isolating (staying at home)?
Have you had had your two doses of vaccination? (You must be 14 Days after your second dose to enter the salon)

If you have not received at least 2 doses of the vaccine, unfortunately we can not take your booking at this time. For clients with a medical exemption please contact us directly for more details on booking an appointment. 

When entering the salon please have your valid Id ( Drivers License/Passport/Health Card ) along with your Vaccine Passport/QR Code. If you can not supply both of these documents on the day of your appointment unfortunately we will have to refuse service, so please come prepared so you can relax and enjoy your service. 

All staff members are fully vaccinated to help create a safer environment during your visit and for an extra layer of safety our team take a rapid test once a week to help prevent the spread of Covid-19.
